Symptom narrative

At 1:38 pm approximately 8 minutes after given the vaccine patient reported dizziness, eyes feeling fuzzy, feeling like she could not breath, and anxiousness. Provider and nurses responded. They had her blow into a paper bag, took blood pressure and pulse, and deemed this to be a panic attack. Hydroxyzine hcl 25 mg tablet (1 tablet for 1 dose) was ordered, dispensed, and administered.
