received only the sterile water component; received only the sterile water component; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of inappropriate preparation of medication in a 45-year-old male patient who received MMR (Priorix) (batch number 4mg3e, expiry date 01-DEC-2026) for prophylaxis. On 08-AUG-2025, the patient received Priorix (intramuscular, left arm). On 08-AUG-2025, an unknown time after receiving Priorix, the patient experienced inappropriate preparation of medication (Verbatim: received only the sterile water component) and inappropriate dose of vaccine administered (Verbatim: received only the sterile water component). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
