Site: Pain at Injection Site-Mild, Additional Details: Requested by my employer to fill out a Vaers form.Although documentation in chart shows MMR not reconstituted, medication was as demonstrated and documented with solution lot and expiration.Documentation was not allowed to be addended. It was a charting error not patient error. Therefore charting remains as not reconstituted although it was.Patient did receive MMR reconstituted and no error in administration or adverse effect occurred to the patient. No pain at site
