Symptom narrative

Clinic medication refrigerator (for vaccine storage) lost network connectivity to temperature alert systems after clinic hours. A temperature excursion occurred during the time of lost connectivity, and no alerts were generated as connectivity was disrupted. The temperature in the refrigerator, with refrigerated vaccines inside, reached freezing conditions for an unclear amount of time overnight within a 12 hour period. HPV vaccine from the refrigerator was administered to the patient before the temperature was found. No adverse effects noted, and the patient was brought back to clinic in the same week for revaccination per guidance from website.
