Patient was seen in HD on 06/16/2025 and given 6 months vaccines DTAP/IPV/HIB/HEPB , PCV20 and Rotavirus. . His name was misspelled in Electronic MEdical record, so vaccines given produced new MCIR record and vaccines given did not merge with previousl y given vaccines. Mom brought him back in Aug for more vaccines and MCIR pulled was wrong spelling so he was given 4th dose of ROTa and 4th dose of PCV20 , within too short of interval.

MOm states that patient was teething at time of getting vaccines in Aug and since then has kept his tongue out of mouth often and has seemed more fussy since then. He is eating , smiling, playing, as he was prior to this, mom does describe him " maybe not using one side of his body as much when he roles over" Family has apt with the PCP today 28 days after vaccine for " good check up' mom also states " I have such anxiety with him after having multiple miscarriages, and high risk pregnancy and delivery
