Symptom narrative

Pt received dose of HepA vaccine from medication refrigerator that had experienced a temperature excursion (too cold) the prior evening. Clinic staff were unaware of the temperature excursion as no alarms were generated due to loss of monitor connectivity. Vaccine administered before excursion was discovered. Pt rescheduled for a repeat vaccination per guidance from agency.
