Symptom narrative

Patient experienced a bruise-like mark on arm soon after vaccine was given and still has a dark mark on skin and tenderness at injection site 5 months after vaccine was administered.

Current illness

Symptomatic cholelithiasis, elevated liver enzymes

Medical history

hypertension, hypothyroidism, osteoporosis, osteoarthritis

Other medications

Gabapentin 300 mg, metoprolol succinate 100 mg, lisinopril/hydrochlorothiazide 20/12.5 mg, levothyroxine 100 mcg, diclofenac 1% gel, amlodipine 5 mg, alendronate 70 mg
