Symptom narrative

Patient returned to the clinic on 9/4/25 for his PPD read. Parent stated that patient had developed a rash starting on 9/3/25. Nursing staff observed flat red areas on both upper extremities, as well as along Patient's flanks. Denies itching, SOB, and nausea. Lungs were clear to auscultation, denies any throat tightness or facial swelling. Parent advised to monitor symptoms and go to the nearest emergency room for evaluation.
