Symptom narrative

I gave a third dose of HPV vaccine when only 2 doses were needed per our protocol and recommendations. The vaccine was not pulled over from system 1 to system 2 so the correct amount of doses were not on the system 2 print out and I over looked the number of doses received on the system 1 sheet provided. No treatment needed. Mother was notified.
