Symptom narrative

Patient originally called and described a local reaction. I told her to monitor it and call me back in a couple days. She said it was red and tender. Patient said it wasn't at the injection site but under/around it. She went to the doctor's office and the doctor office called me saying nearly a month later, her arm was still painful.
