Symptom narrative

Patient called our office on 09-08-25 about 6 hours after receiving vaccine complaining of having hives all over body and hoarse voice. She took a benadryl prior to calling. Later that day, developed low grade fever, chills & nausea. Started wheezing the same day. Used her proventil inhaler
