Symptom narrative

Patient wanted to let us know he is still having some pain in his RIGHT arm. He is having painful limitation of motion when he raises his arm. He has not seen a doctor yet in hopes that it would go away, but vaccine was 3 months ago. Vaccine logged as given in the LEFT arm for administration site, but patient says he got it in the right arm.
