Symptom narrative

Child was Due for DTaP #4 and Polio #4, Kinrix= DTaP/IPV was used in draw up error (was not DTaP #5 and Polio#4 or 5) was supposed to be DTaP only- not a combined product. That vaccine was administered along with Polio#4 in a separate dose along with MMRV#2 and Hep A #2. The error was realized right away. Kinrix was not supposed to be used, it was supposed to be a regular DTaP by itself which would have not caused this vaccine error to occur. There were no signs or symptoms of adverse reaction, no fever, no injection site redness or rash. The child was fine, parents understood that wrong DTaP product was administered in error, and 2 polio doses were inadvertently given and one of them would not count; however, the child did receive what was needed for school. After at least 10-15 minutes family left. Child was not injured or ill.
