Symptom narrative

Client came in for vaccines and this nurse administered a pediatric dose in error. Client advised of vaccine error and that I will have to administer a second half dose for a full 1 ml adult dose. Client voiced understanding and agreed with plan. The additional pediatric dose was administered for a completed vaccine. Client monitored for 15 minutes and no adverse reaction was noted.
