Symptom narrative

First administered Spikevax, then 1 minute later administered Prevnar but this was an error due to it being left over from a previous appointment and within reach whereas patient was expecting Flucelvax. Patient immediately told what was given, that's it's completely safe, patient declined getting the correct vaccine in addition. Her reaction turned into lightheadedness after a few minutes, including falling down upon exiting vaccine room. Patient was helped into a chair, given CocaCola and water, and improved and left after 15 minutes. Patient declined 911 service. All team members/managers notified immediately and steps taken according to procedure.
