Symptom narrative

Vaccine/component omission - Diluent given without the vaccine: only the adjuvant suspension component (vial 1 of 2) was administered to the patient. It was not reconstituted with the Lyophilized gE Antigen component (vial 2 of 2) as indicated. The patient did not receive the entire dosage of the Shingrix as a result. No Adverse reactions were reported by the patient.
