Symptom narrative

Pt had a bruise of upper deltoid believed to be due to the Boostrix. Below the bruise was a rash that had been getting larger over the past few days, warm to the touch and with an area of more firm tissue below the skin.

On tamoxifen post breast carcinoma

Tamoxifen, Levothyroxin, Acyclovir,

No tests performed, I recommended pt see her doctor and to inform them of the situation. Later the patient's Dr sent in 1 Rx for prednisone 20mg and another prescription for Cephalexin 500mg
