Symptom narrative

Patient was to receive the ProQuad vaccine at today's visit - dose and vial were checked by two RN's prior to administration - following administration, it was noted that the vaccine had expired on 08/26/2025. I called and spoke with employee at company - who stated that based on the information provided, they could support the potency of the vaccine. Called patient mother to make her aware - no answer - left voicemail to return a call to the office
