Symptom narrative

Patient reported feeling sweaty and dizzy at 11:40 a.m. Patient was evaluated by Dr. and stayed for observation. Patient's vital signs were taken and offered juice and crackers. Patient only took a couple bites of crackers and sips of water. At 12:10 p.m. patient reported feeling better and denied feeling dizzy and sweaty.
