Symptom narrative

Pt was started on Metronidazole cream (diagnosis code: Rosacea) and Cyclosporine eye drops the same day as she was given the 4 vaccines. Patient states she developed papules on her neck and chest two days after which have not gone away. Her PCP prescribed her Hydrocortisone cream because it was "a little itchy". She presented today to the pharmacy to report these events to us (one month later). Patient showed me the papules, advised to see PCP as soon as possible.
