Symptom narrative

Pt received 3 shots and was asked the routine questions and were answered appropriately. After shots were administered, Technician stepped away to computer and heard the Pt fall and saw the Pt hit the cabinet and slide down onto the floor. Technician checked for Pt responsiveness and Pt immediately awoke. Rapid Response was called and treatment was provided to the Pt.
