Symptom narrative

Patient went in for his Primary yearly check up and was given a flu shot. 12 Hours later he got a very bad headache and fell over onto the floor. Ambulance was called and at 1:30 am patient was unresponsive and never recovered. He had a massive brain bleed. He was put on a ventilator and died 24 hours later.

Current illness

Diabetes, tecacardia, a fib.

Medical history

Yes diabetic.

Other medications

Eliquis 5mg, DULoxetine 60mg, gabapentin 600mg, insulin 100 unit ml, empagliflozin 10mg

Allergies

Azithrmycin, erythromycin, nsaids.
