Symptom narrative

Approximately 5 minutes after receiving vaccine patient c/o feeling dizzy/lightheaded. Denied nausea/vomiting. Denied sweatiness/warm. Immediately elevated patient legs, provided patient with water which she drank. BP 97/57 P97. Constant observation of RN. Patient stated she was feeling better. Spoke with Supervising provider who advised to give more juice/water and recheck BP. Upon returning to the room patient and parent had left telling RN she was feeling better and they were going to get something to eat.
