Symptom narrative

During the administration of the 11-year-old immunizations, the patient was extremely combative and uncooperative. Due to the level of resistance, an additional clinical assistant was called in to help safely restrain the patient for the vaccine administration. While attempting to manage the situation, the assisting clinical assistant removed a Shingrix vaccine from her pocket and inadvertently placed it in the same tray/area as the scheduled vaccines intended for the patient. Due to the chaotic circumstances and lack of clear labeling or communication, the Shingrix vaccine was mistakenly administered to the patient without my knowledge.
