Symptom narrative

Systemic: Dizziness / Lightheadness-Severe, Systemic: Fainting / Unresponsive-Severe, Systemic: Flushed / Sweating-Medium, Systemic: Seizure-Severe, Additional Details: Patient received vaccine and with in 2-5 mintes got light headed and passed out. Patient had slight convulsions. Called 911, EMS checked vitals, upon pricking finger for blood gucose check, patient fainted and convulsed again. EMS took patient to hospital.
