Symptom narrative

Site: Pain at Injection Site-Medium, Site: Shoulder Joint Injury (prolonged pain, tingling, etc.)-Medium, Systemic: Numbness (specify: facial area, extremities)-Medium, Additional Details: Patient felt rph gave shot too low, complained of numbness still of extremity, arm heavy, pain and numbness at base of thumb. I told her to contact her PCP and relay this information for additional treatment if necessary.
