Symptom narrative

This occurred over the weekend when I was not here. Monday morning the technician told me that over the weekend she had a vaccination error. She said that she put a needle on a flu shot and stuck the patient then realized the needle was too long and removed it from the patients arm and went and put a shorter needle on the flu shot and went back and re-stuck the patient and gave him the flu shot. She said she reported to agency but not to the pharmacist on duty because of his attitude towards her. She said she was scared to tell him. I am reporting to vaers because I do not believe that anyone else has. The patient did not have an adverse reaction that we know of at this time, but he was stuck twice which is a vaccination error.
