Symptom narrative

Patient noticed erythema/redness at the site of injection on his left arm. When I reviewed the reaction he experienced I noticed that the site of injection was slightly lower than where his deltoid muscle is located which may have contributed to the adverse event experienced. Patient also describes the sensation as itchy and irritated. Patient lastly also described that the injection site was "hard" or felt as though a nodule resulted from vaccination.
