Patient was given Pentacel by the Medical Assistant instead of the Menquadfi that was ordered. Provider was informed. Medical Assistant informed the patient of the event. Patient was scheduled per provider approval to return on 9/11/25 for the Menquadfi vaccine. No adverse reaction reported.
