Symptom narrative

Pt describes urinary incontinence starting 1 day post vaccination that she could not controlled- pt states stoppeed next day; She also states she had local injection site reacion- raised, red area, itchy--by 09/12-when she presented to pharmacy- arm was very light pink-you could see the red, irritated local area clearing; pt stated no longer itching
