Symptom narrative

Patient reported on 9/17/25 that on shortly after getting her vaccination on 9/12/25 it began to itch. She did not report this to the pharmacy staff but went home and put hydrocortisone cream on the injection site. She reports it was sore to touch, began to swell and became red. Today she is still reporting itchiness. Encouraged patient to contact her PCP.
