Symptom narrative

Pt. was to get varicella and HPV vaccinations. One dose of HPV was accidently mixed with Varivax instead of sterile diluent. This mistake was not realized until that mixture of the 2 vaccines, and also a separate HPV was given to pt. Pt. did not report any adverse effects from this.
