Symptom narrative

patient states she is feeling malaise after receiving her second meningococcal vaccine on 9/15/25. temp was normal 98.4 (oral). rash present on upper left arm and is approximately 30 mm x 40 mm. Raised and red with no blistering, pustules, or bleeding. States her arm is extremely sore/painful. Limited ROM due to discomfort. No circulatory issues, cap refill < 3 seconds on all digits, bilateral grips on both hands equal, pulses present and equal on both wrists. No shortness of breath reported. Benadryl given.
