Dr. ordered the HIB vaccine. Medical assistant administered the HPV vaccine. Mother was notified by Dr. at approximately 1 pm. Dad called Dr. about 2 hours later and reported that patient seemed uncomfortable and complained of pain at the injection site. No other adverse effects reported by parents. Dad expressed concern and said he was going to bring patient to ED. Dr. informed the ED. Dr. was told by the ED that patient never went to ED. Dr. called Dad's cell again and left a message requesting a call back. Dr. never received a response.
