Symptom narrative

During a Physical Fitness Test, I experienced a syncope episode and lost consciousness. After this initial incident, I began noticing recurrent symptoms whenever I attempted to exercise. Each time I engaged in physical activity, I would develop pre-syncope sensations such as lightheadedness, dizziness, and a rapid increase in heart rate. Over time, these symptoms worsened to the point that my heart rate would skyrocket during even moderate exercise, and I would frequently feel as if I were about to pass out?or actually lose consciousness?while working out. Because of these repeated episodes, I am now unable to exercise safely without experiencing near-syncope or fainting.
