Symptom narrative

patient given vaccine and monitored after, was released to leave the office. On way out of office had an unwitnessed fall. Was sent to ER where they said he had a syncopal episode.

Current illness

drug usage, Heart problems.

Medical history

Diabetes with toe amputation, HTN, CKD, CHF, HLP, HX DVT, Memory impairment, Adrenal adenoma, Neuropathy, Mitral valve regurgitation, Ventricular tachycardia.

Other medications

Low dose aspirin, Atorvastatin, Gabapentin, Losartan, Metoprolol, Xarelto, Spironolactone, flomax, Torsemide.
