Symptom narrative

Onset of pain following pneumococcal vaccine administration on 09/05/25 with pain persisting 2 weeks later (today appointment for evaluation on 09/18/25), affecting mobility and use of arm due to pain and tenderness with active and passive ROM. Patient has mild erythema of L upper deltoid region toward glenohumeral joint, crepitus of LUE > RUE, and tenderness/pain with active and passive ROM of LUE.

No other documented illnesses up to vaccination time

Medical history

Meningioma; essential hypertension; pacemaker; AV block Mobitz 2; complete AV block; bilateral sensorineural hearing loss; age-related osteoporosis without current pathological fracture

Alendronate 70mg 1 tablet PO weekly; amlodipine 10mg 1 tablet PO daily; atorvastatin 20mg 1 tablet PO daily; lisinopril 10mg 1 tablet PO daily; acetaminophen 325mg PO PRN; calcium carbonate 300mg daily; cyanocobalamin 500mcg 1 tablets daily
