Symptom narrative

States that patient shortly after vaccine administration presented with fevers and body aches which led them to seek medical provider care after no improvements in symptoms after a few days and she was recommended to take patient to Hospital for additional screening and care. Patient has been evaluated by facility report indicates patient was diagnosed with: Acute Hepatitis B17.9, Abdominal pain, upper R78.09-R10.10, and Altered bowel habits R19.4.
