Patient received the Meningococal vaccine IM in LD the HPV vaccine IM in the RD. After receiving the HPV vaccine the patient got pale, diaphoretic, holding his breath, blurred vision (afterwards he said everything looked like a checkerboard), and nausea. BP 96/43, HR 65 and twenty minutes later BP 107/65, HR 64. Applied icepack and fan, removed shirt. Once he improved a snack and water were given. At 4:03 patient reports he is feeling like himself again. He left the clinic driving and his mom followed him in her vehicle. Patient alert, oriented.
