During certification, immunization records were reviewed, and it was noted that the child needed four immunizations. Per staff, while in the lab mother was asked if she would like the child to receive the shots. The mother did not respond initially. Once they returned to the back, staff again informed mother that the child needed four shots and asked if she wanted the child to receive them today. She stated "yes." Staff then asked if it would be acceptable for the father to accompany the child for the immunizations while she remained to complete questions. The mother agreed. Staff escorted both the father and the child to the waiting room to wait for the next nurse available. I verified child's immunization record before calling child back to the clinic area. Child was due for DTap, Polio, MMR, and Varicella according to the record. VIS sheets were given to Dad. Went over possible reactions of side effects that could occur. Dad voiced understanding and agreed to all vaccines being given. After administering vaccines, Proquad and Quadracel, it was noted in immunization record that the patient had already received vaccines today. Parent did not inform nurse during education about vaccines that child had received vaccines earlier today. Information was not entered into immunization record at the time of the visit from the outside provider. Patient received immunizations today from ADH and MD office.
