Symptom narrative

Was sent home from clinic. He has missed two appointments d/t high blood pressure, pt was in visible distress, autonomic response as evidence from sweating profusely, BP taken 166/94 and 132b/m (resting BP around 128/68 and 80HR). Patient was instructed to sit down throughout session and was sent home early.
