Symptom narrative

Child received Vaxelis that was not needed. The immunization record from out of country had not been entered into system and it appeared that child was behind on vaccines when this was not the case. No adverse effect noted in child. No symptoms, no swelling or redness at injection site. Remained in office >20 minutes. There was no treatment given, mother was made aware. Dr was made aware.
