Symptom narrative

Patient returned to pharmacy today to say that she had a reaction to her immunization on Tuesday. Patient complained of increased Heart Rate, shortness of breath, sweating, fever and injection site reaction that included swollen arm, redness, warm to touch and tender. Patient went to the PCP office due to her reactions on wednesday , then went back on thursday and friday. Her heart rate was high at 130, and they had her hooked up to an EKG for an hour to monitor her. Pt also states that they gave her metoprolol help to lower her heart rate.
