Symptom narrative

The child was inadvertently given both a Proquad (MMRV) and a Varicella vaccine during same vaccination visit. The child did not display any adverse effects. There were no symptoms and no signs of redness or swelling at injection site. Doctor was made aware of error. Mother has been notified.
