Symptom narrative

Patient endorses that she received 2 IM injections in her left arm and one IM injection to her right arm. Patient states within a few hours she developed right arm pain with heaviness and weakness to the affected extremity. Patient endorses transient right leg pain and heaviness that started after 24 hours that resolved within 72 hours of immunization. Patient is now having excruciating pain with heaviness and weakness to affected right upper extremity 5 days after vaccination.
