Symptom narrative

Systemic: Confusion-Mild, Systemic: Dizziness / Lightheadness-Medium, Systemic: Fainting / Unresponsive-Medium, Systemic: Flushed / Sweating-Mild, Systemic: Shakiness-Mild, Additional Details: Patient bleed from injection site after admin caused from tensing up arm. After 2-3 min after injection, patient fainted and hit head on vaccine privacy curtain. Patient lost consciousness for few seconds, meanwhile I propped patient in rescue position. When patient came to, extremities were cold and mouth hurt from biting cheek/tongue. Call 911, EMS came promptly and took patient to (withheld name) ER.
