Systemic: Dizziness / Lightheadness-Mild, Systemic: Exhaustion / Lethargy-Mild, Systemic: Hypotension-Mild, Systemic: Nausea-Mild, Systemic: Vomiting-Mild, Additional Details: PT stated that he gets very nervous with shots, also said he didnt eat or drink much before the vaccine. The Imz tech used correct vaccines/technique. After the vaccine was given, pt stated that he felt dizzy and looked pale. that's when the tech called the RPH. RPH went over to access the situation. Water was given to Pt. The RPH took pt's vitals BP: 97/42 and HR was 66, Pt started to vomit, That is when the RPH called EMS and pt went to ER.
