Symptom narrative

Site: Pain at Injection Site-Mild, Systemic: Body Aches Generalized-Mild, Systemic: Headache-Mild, Additional Details: Patient had previously recieved the COVID-19 vaccination on 09/07/2025. The patient came in for a flu vaccination on 9/15/2025 at 11am; however, a COVID-19 was mistakenly given to the patient. The primary care doctor of the patient, was informed of the incident at 12:32 pm the same day. The nurse of the primary care doctor, stated they will follow up in 24 hours to determine if further action is needed. The pharmacist, followed up on 09/16/2025 at 9:30 am.
