Symptom narrative

Site: Redness at Injection Site-Mild, Additional Details: incomplete dose given. Pt was getting 2 vaccines, the 1st was covid, administered in L thigh (pt was sitting in his father's lap). Then after the start of administration of second vaccine (flu in the R thigh) pt jerked his leg up and down, so the needle came out, I was still pushing the vaccine in so some of the vaccine did come out on his thigh (hard to quantify), and then a few scrapes happened with tip of needle on patient's thigh. Called (withheld) & pedi office, think at least 50% given, no repeat
