My daughter recently went in for a routine physical and was supposed to receive the MCV4 vaccine. Instead, she was mistakenly given the DTaP vaccine. A few days later, the clinic called to inform me of the error. While my daughter has not had any adverse effects, I want this incident formally documented to help prevent similar mistakes in the future. If this had involved a stronger or less appropriate vaccine, the outcome could have been unpredictable and potentially harmful. I was also concerned that the nurse did not seem very apologetic, which makes me worry that the seriousness of this error may not be fully recognized. I hope steps are taken to ensure this does not happen to other patients.
